The National Health Fund (NFZ) has amended its regulation on the conditions for concluding and implementing contracts for the provision of healthcare services, including primary care. It will come into effect on September 12th.
As the Fund reminds in a press release, tasks performed by coordinators in primary healthcare facilities have been covered by a capitation rate since 2021. This rate is payable for every patient over 24 years of age who is registered with a primary healthcare provider.
Following the implementation of coordinated care in primary healthcare (POZ) – from October 2022 – POZ facilities that have expanded their contracts to include the scope of services related to coordination have had their capitation rate for coordinator tasks increased by 25%. Furthermore, for these facilities, it covers patients from 18 years of age registered with a given POZ.
From October 1, 2025, the group of patients for whom the coordinator will receive remuneration will expand to include children.
Monthly allowance for primary healthcare institutions that will enter into coordinationThe monthly lump sum additionally received by primary healthcare facilities with coordinated care will be paid from 1 October 2025 for 6 months from the signing of the agreement for the introduction of coordinated care to a specific primary healthcare facility.
"We are therefore maintaining the incentive nature of the coordinator's lump sum. This was precisely the goal behind the introduction of this allowance for primary healthcare providers in 2023, as it is intended to encourage new primary healthcare providers to join the coordination effort. From the outset, we have been clear and open about the fact that this allowance is granted for a fixed period," emphasizes Paweł Florek , Director of the Social Communication and Promotion Office at the National Health Fund Headquarters.
The amount of this lump sum is currently approximately PLN 7,700 per month for primary healthcare facilities that decide to enter coordinated care and have up to 5,000 registered patients under their care.
Coordinated care in primary care. Additional tasks for additional resourcesCoordinated care at primary care centers is additionally financed by the National Health Fund. This includes not only higher remuneration for the coordinator, but also separate payments for a broader package of patient testing, specialist consultations, comprehensive consultations with primary care physicians, and educational and dietary advice, the Fund points out.
He adds that separate financing (apart from the capitation rate) for health services enables comprehensive treatment of a patient with a chronic disease at the primary health care level.
"In 2024, we paid over PLN 546 million to primary healthcare facilities under the coordinated care budget. In the first quarter of this year, this figure rose to PLN 148.2 million," reports Paweł Florek.
According to the latest data, the National Health Fund (NFZ) has signed nearly 3,000 contracts for the implementation of coordinated care in primary care settings. This represents approximately 48% of all primary care contracts . However, from a population perspective, nearly 20.6 million patients registered with primary care settings are covered by coordinated care, representing 60%.
